How to make an appointment: We ask that patients see their General Practitioner prior to making an appointment with Dr Walkom in order to obtain a referral. This is required by Medicare Australia in order to receive the maximum rebate on your consultation cost. To make an appointment contact our reception staff Telephone: 02 6552 6344 Monday - Friday 9.00am to 5.00pm
Please note: All appointments must be made over the telephone.
Cancellation of appointment: Dr Walkom requires a minimum of 24hrs notice of cancellation of appointment. Patients who fail to give appropriate notice will incur a non-attendance fee. Unfortunately no further appointments will be scheduled until this fee has been paid in full.
What to bring to your appointment: When attending your appointment at Dr Walkoms office please ensure you bring relevant health cards (Medicare card, Veterans Affairs card, Private Health card, etc). At your initial consultation you should also bring your referral from your doctor/specialist if they have not already sent this to our rooms. If you have had any ultrasounds/x-rays done could you please bring these with you to your appointment. Copies of any pathology (blood tests) would also be beneficial and your GP may arrange for copies to be sent through prior to your consultation. 

Antenatal patients will need to bring their Obstetric Card if they have one.
Patient Registration (Personal Details): All new patients will be requested to complete a Patient Registration Form, which asks pertinent questions such as name, date of birth, address and health insurance details etc. This information will be entered onto our secure patient database and a hardcopy will be placed in your file. Patients will also be requested to sign a Privacy Information & Consent Form.
